QEC operates as a continuous measure-decode-correct cycle: ancilla qubits are read out, a decoder infers errors from the resulting syndromes, and corrections are applied before the next round begins. Within this loop, readout occupies a uniquely critical role, as it is the sole source of ground truth available to the decoder.
